Cancellations
We understand that emergencies do arise, however, there is often a waiting period to see the doctor, so we request notice of cancellation as soon as possible. We reserve the right to charge a fee for one not canceling an appointment within 24 hours of the scheduled time and/or failing to show up at an appointed time. Any cancellations made less than 24 hours prior to a therapy appointment can be fined, and two no-shows may result in termination from the therapy.
Since psychological evaluations are usually completed in one session, we are only able to accommodate our regular office hours and days, and this varies depending on location. With that being said, we ask that if you need to cancel or reschedule an evaluation, you do so as early as possible and we will try to find another time to reschedule.
Please note that insurance companies typically do not provide reimbursement for “late cancellation” or “no show” sessions. Therefore, you will be responsible for the portion of the fee as described above.
Policies & Fees
Fees are due at the time of service and some fees may be billed later for psychological testing. Fees vary depending on the insurance, however, we do NOT guarantee insurance fees as this may change based on the explanation of benefits from the insurance company. You are responsible for knowing your insurance policy and fees. Non-payment of services will result in being sent to a collection agency and additional fees.
Again, cancelations prior to 24 hours are subject to a cancelation fee, and two or more no-shows may result in termination from therapy.
Billing & Payments
If you have insurance, we will bill the insurance provider if we can verify coverage at the time of your first visit. Otherwise, we will expect full payment from you. You will be responsible for any co-pay or deductible your insurance provider requires. You are responsible for any co-pays and deductibles at the time of service.
Sources of Payment
Thurman Pychological LLC sources of payment include:
- Cash/Check
- HSA plans
- Credit cards (Visa, Mastercard, Discover, and American Express)
- Workers Compensation – If you were injured at work, you may be eligible for compensation from your employer’s “work comp” policy.
Accepted Insurance
Thurman Psychological LLC accepts a variety of insurance. If you do not see your provider listed below, please contact us to ensure acceptance of insurance. (Please note that some insurance companies require pre-authorization prior to testing).
Referral Agency Contracts
Thurman Psychological LLC accepts several agency contracts. Referrals are accepted from a variety of agencies including schools, workplaces, DHS or CPS, and law enforcement agencies.
In certain situations, a release of information may be necessary, and in other situations, the referral agency may pay for the evaluation and request the evaluation to assist in help with employment, school services, etc. In most situations, HIPPAA and privacy apply but there may be some legal cases in which the third party is entitled to the information in the evaluation.
