Your First Visit
We thank you for choosing Thurman Psychological! Completion of forms prior to the appointment allows for the full amount of time with the doctor and utilizes time effectively. Identification and proof of insurance are required. On your first visit, be sure to bring:
- Insurance information
- A list of any medications you may be taking
- Contact information for any other healthcare providers you want us to coordinate with
*Agencies that refer minors must have paperwork signed and completed by the appropriate guardian prior to the appointment.
